Form 1583

What is it for?

Form 1583 is a permission slip that allows a third-party company (like Shipping Made Easy) to receive and handle your mail. 

Why do you need it?

When you rent a Private Mailbox or use a mail forwarding service, the USPS requires that they verify who you are to make sure your mail isn’t being intercepted fraudulently. Form 1583 helps USPS confirm your identity and gives the mail handler permission to accept your mail on your behalf.

What do you need to do?

1. Fill out the form (online or in person): You provide your full name, address, and the name of the company you’re giving permission to handle your mail (Shipping Made Easy).

2. Submit Identification: You’ll need to provide two forms of ID (like a driver’s license, passport, or vehicle registration) to prove your identity. One of these IDs needs to be a government-issued photo ID.

3. Sign the form: You sign to give permission for the mail to be received by Shipping Made Easy.
